Cholinesterase is an enzyme which catalyses the hydrolysis of the neurotransmitter acetylcholine into choline and acetic acid, a reaction necessary to allow a cholinergic neurone to return to its resting state after activation.

Measurements of cholinesterase are frequently performed as an aid in monitoring exposure to organophosphate insecticides and other anticholinesterase compounds. Usually this involves making consecutive measurements on a person over a period of time. Increased cholinesterase activities closely related to lipoproteins metabolism are observed in hyperlipoproteinemic patients, and its use as an indicator of cardiovascular diseases has been proposed. Decreased cholinesterase activity is found in prolonged liver diseases and may be a reliable sign of impaired protein synthesis or decreased liver cell mass, or both.
