Vancomycin is a relatively small glycoprotein derived from Nocardia Orientalis. It is active against most gram-positive bacteria including Streptococci, Corynebacteria, Clostridia, Listeria, and Bacillus species. Vancomycin- resistant mutants are rare, except for vancomycin resistant Enterococcus. Intravenously, it is used to treat systemic infections such as endocarditis caused by methicillin-resistant (MR) Staph aureus, meningitis caused by Flavobacteria, CNS shunt infection caused by MR Staph, Ampicillin- resistant enterococcal infections, infections caused by organisms susceptible only to vancomycin such as Corynebacterium jeikium, prophylaxis for major surgical procedures for implantation of prosthetic materials or devices at hospitals with a high rate of infections due to MR Staph and in patients with true ß-lactam allergy. Orally, it is used only for the treatment of antibiotic-induced pseudomembranous colitis caused by Clostridium Difficile.
دواعي الإستعمال
Measurement of Vancomycin levels are performed to ensure concentrations are within therapeutic range and avoid drug toxicity. Side effects include nephrotoxicity and ototoxicity which may occur in < 1% of patients. Neutropenia, thrombocytopenia and hypersensitivity are very rare.
