Indications
Urinary free cortisol is proposed as a test of adrenal function. It is considered the best screening test of endogenous hyper-cortisolism, assuming complete collection of urine. The test has superseded measurements of the excretion of 17-hydroxy-corticosteroids and 17-ketosteroids, which are dependent on body weight and creatinine clearance, and are less sensitive. Increased urinary free cortisol levels have been reported in patients with essential hypertension.
